I am hereby authorised Om Space Sdn Bhd (“The Company”) to act on behalf and represent me for the hypnotherapist membership and insurance renewal process. This authorisation is effective from the form submission date with proof of consent and signature.

Terms and Condition:
1. The membership and insurance information may change from time to time depending on the particular association and insurance company update. Kindly take initiative to follow up with the particular party if you have any questions.
2. Om Space Academy has no obligation in updating the graduates on renewing the membership and insurance policy on the second graduation year onward.
3. For further assistance in renewing the membership and insurance policy, Om Space Academy is providing the administration service of renewal process with fixed price and administration fee.
4. The administration fee is non-refundable if the renewal transaction and process is unsuccessful due to any reason.
5. The graduates are fully responsible in any incidents and accidents which are included and not limited to:
• Missing of parcel
• Wrong personal information
• Exceed of renewal expiry date
• Unqualified condition by the association and insurance company
• Any accident happened during the renewal process
6. Any wrong information found on certificate, card and policy, graduates may declare it to the respective association or company and request them for amendment.
7. The Administration Department may process your renewal in 7 - 14 business days.
